At the time of your initial contact with our office for a new appointment, you will be asked about your insurance plan. You will speak with a billing coordinator who will complete a new patient registration, which will include information about the health insurance plan, the insured, policy/group numbers and the plan’s address.
You will be asked to provide your social security number so a medical record can be established for you. It is your responsibility to notify the office any time there is a change in your insurance plan or coverage. You will be asked to bring your new insurance card to the office, so it can be electronically scanned into the system for billing accuracy.
We participate in the Worker’s Compensation program. Please inform us at the time of appointment scheduling that you are being seen as a result of a work related injury. For us to file under Worker’s Compensation, you will be required to bring the following information to the office at the time of your initial visit:
Any diagnostic study or treatment plan will be subject to the Worker’s Compensation Board’s approval. The office will submit the required documentation to expedite this process.
We participate in the Michigan No Fault program. Please inform us at the time of appointment scheduling that you are being seen as a result of a motor vehicle accident. For us to file under No Fault, you will be required to bring the following information to the office at the time of your initial visit:
If your Michigan No Fault benefits have been exhausted, we require an official letter from the insurance carrier stating this so we may bill your private health insurance for services provided.
Regrettably, we do not accept liens or settlement payments from attorneys for services rendered.
