Visit Experience

It is important to arrive at the office prepared for your appointment, including information you need to bring with you.

We contact you with an appointment reminder; however, it is your responsibility to notify us of a cancellation at least 24 hours in advance.

Co-payments and deductibles are expected to be paid at the time services are provided.

We use an answering service after hours to answer urgent calls.

You will be asked about your insurance plan and to bring your new insurance card to the office.

If your health insurance plan requires a referral to see a specialist, it is your responsibility to request that referral well in advance of your appointment.

During your initial visit, you will be asked to complete a medical information form where you can indicate those persons to whom we may disclose information.
